Question

I have a 22 year old gelding paint horse who has an yeast ...

I have a 22 year old gelding paint horse who has an yeast infection coming out from the inside of his sheath! not the outside, it is coming from within in body. no one seems to know how to cure it. I ask several vets. all of there ideas didn't work. nor do they understand that the yeast is coming from inside of him and not on the out side of his sheath. e-mail me and I will give you full details on what I have tried over the last three years.He act like sometimes he doesn't want me to get on him cause his kidneys hurt. this horse has a high tolerance of pain. so when he does this I"m thinking something in his kidney that is infected. Isw there a vet out there who can help me with this problem? before I have to put my horse down.????Bertag05@yahoo.com

Have your vets done any urinalysis and/or bloodwork to check kidney function? What exactly are you seeing that indicates a kidney or yeast infection problem?

yes the vet did both said the kidney was enlarge alittle but nothing came back as unnormal. he has yeast from his sheath everyday even after I clean it a day later. his kidney are normal as of now but he acts like he doesn't want me to put his saddle on .I have'd this horse for she laid back and doesn't care what you do with him.eventeen years and he doesn't do this normally.thank you , alberta

Did your vet rectally palpate to check kidney size and/or do an ultrasound since you said he thought it was a little enlarged? What qualities of the discharge make you think it is a yeast infection (color. smell, consistency, etc)? Does he strain to urinate, urinate more frequently than normal, unusual urine appearance? Rub his butt on the fence indicating sheath irritation? What are you using to clean his sheath? How long has this been going on?

The vet did a palpate-no ultrasound said it felt alittle enlarge-ran some tests said they were normal range for his age?????I don't know what that is.I sent a sample of the smelly off-white cottage cheese looking stuff to a&m university. they reply back yes it is a yeast infection,but no comment on curing it.???this was 3 to 4 years ago.he does rub his tail like it is itching sometimes to the point where he rubs it raw.I work 50 to 60 hours a week I don't get to notice him urine too much but when I do it don't seem like he has trouble.I clean him tonite with novan scrub cleaner, dry it with a towel put novan lotion inside as much and outside and also put penacillin on the very outside of bottom of sheath.the outside sometimes if I don't keep it clean get's crusted over.I try not to clean to much cause I fear I am tappering with mothernature natural stuff.he has a open red sore on inside of his flap? i put penacillin on italso cause he move away and he usually lets me do what I want with him. then I notice a little red sore.hope this help you. wish I could talk on phone cause I am not sure I am getting across what I want to you. sorry,alberta

So it is sounding to me as an external problem, not something that is coming from is urinary tract (kidneys, bladder, urethra). You are right that over-cleaning will upset the normal flora of the sheath. Penicillin will not kill off the yeast and will kill off the good bacteria that keep yeast in check. Have you tried betadine scrub instead of nolvasan? It may work better against yeast. There is a sheath cleaner called Excalibur that has tea tree oil (which has anti-yeast properties) as an active ingredient. You may need to use medications specific to yeast control - since there is nothing that I know of that is labeled specifically for the horse's sheath it would have to be an off-label use. One possibility is Malaseb shampoo, towelettes or spray which works great on dogs with yeast skin infections. Also possibly a yeast infection suppository treatment for women may be effective (such as Monistat which has miconazole). Ask your vet if there are any contraindications for use of these unusual treatments in your particular horse.
