I have a long hair chihuahua that is 4 years old he has a bald spot on his back that he has had since he was a puppy. I t has dry flaky skin on it and small black spots on it also. I have squeezed them and a small black thing comes out that looks like a sliver.   Any hair that does grow on the spot is dark and course and very sparse. Any suggestions?

It sounds like it could be a sebaceous or follicular cyst.

You can try wiping the area with a benzoil peroxide pad and then using neosporin or triple antibiotic ointment on it to see if that clears it up. If the skin looks like its becoming sore discontinue the wipe and just use the ointment.
Or you can see if a veterinary dermatologist will give you more help than your vet has
