Pomeranian w/ pain when lowering head/neck

Our 15 yr old Pomeranian is crying out in pain when trying to lower his head/neck to eat or drink. He can however eat/drink just fine when pan is raised and no head lowering required. What could this be?

nothing other than raising bowls so he doesn't have to bend
