I'm not going to go thru the usual questions, such as are you pre-treating your water because if they've been happy for three years, obviously you're doing something right. What kind of substrate are you using? When they bloat it can be either from an impaction or a bacterial infection/Dropsy.
If "she" ate some of the substrate or ate something too big (what are you feeding?) it could be an impaction.
Dropsy can be caused by chlorine in the water, poor diet or bacteria...no one knows the exact cause. Dropsy can be treated (with baytril injections), but it's touch and go whether they will make it through that, it's usually fatal even with the treatment. (On the other hand, my friends vet recommended euthanizing hers after 10 baytril treatments her toad was still bloated, but they didn't give up and a week and a half later he was fine. We were surprised, but happy.)

In the meantime, you can try soaking her in lukewarm water in case of constipation or a blockage, it may help. You may also want to consider separating her from the other.
To answer your other question, fbt do not get pregnant or "gravid" in the typical way, they do carry unfertilized eggs inside them but it wouldn't show in this way. The fertilizing happens outside the body when they lay the eggs.
