Question

I have a 15 year old Arab gelding who is a bit overweight ...

I have a 15 year old Arab gelding who is a bit overweight and his sheath seems enlarged. he does't seem to have any beans but his inner sheath seems dirty. I have cleaned it with 'sheath cleaner' but that really seemed to inflame the whole area. Could it be that he's just fatter and is carrying some extra fat in this area? So, how to clean it gently and is this a symptom of a bigger problem?

I don't think he is carrying any extra fat in that area, as that is primarily muscles. I do believe that your horse had a reaction to the chemicals in the sheath cleaner. It is actually recommended that just warm water is used to clean the sheath. Using a chemical based product increases the chances of infections and washes away the good bacteria which can also open him up for infections, so, just use plain warm water to do this. I would give your Equine Vet a call, just incase there is an infection as the area is inflammed and you don't want this to turn into something extreme.
