I have a 13 yr old female chow. Recently I noticed that one of her nipples seemed enlarged. She is spayed. She appears to be producing milk, as my male chow is constantly "cleaning" this area. It has become more enlarged, and she refuses to allow me anywhere near this area. I was wondering what this could be, and also, what I need to do. I have kept an antibiotic spray on the area (while she is sleeping and will allow me near this area). Please advise. Thank you.

Keeping male chow away to keep from licking the area, antibiotic ointments/sprays/cleaning area, etc.

You need to have the vet look at her. This could be a mammary tumor and removing it might be the best thing to do.
Just like in humans mammary discharge and something that is sore and doesn't heal may indicate a tumor including cancer.

What else other than a mammary tumour could it be? She has always been in really good health and has never had any problems with her routine bloodwork that would indicate cancer. I've seen tumors before, - it just doesn't look like a tumor at all.

Thats the most likely thing - if its up above a milk duct it could be irritating the gland enough to produce 'milk'.
Another possibility is an inguinal hernia under the gland causing similar type irritation.
I've seen both do what it sounds like you are describing.
