Liver problems?

Thanks for taking the time to give me some answers.

I have a 13 yr old (approx.) female cat, who has always been indoors, though she got out once for 3 days. I am not sure her breed, but I know she is a mix of calico and something else (that I thought was siamese, but not sure). She is about 13-15 pounds and has always been a chunky kitty since she got spayed.

Her medical issues over the years:
-Ear problems that was diagnosed as "protein buildup due to allergies". I just have to have them frequently cleaned out and as of the last year she has gotten mild doses of cortizone shots that have helped her ears (2 times total).

-She has always had tummy problems that comes and goes. She tends to eat very fast and vomit her food and then be hungry again. I know a lot of cats throw up from hair, but she has been excessive and never has hair balls in the vomit. I have thought it is stress, she seems to have a nervous tummy.

Just 2 days ago I came home to find her under the bed and wouldn't come out without a little force. She is always a very friendly/lovable cat and always greets me at the door and always has to be around people, so I knew something was wrong. Then I realized she had puked like 4-5 times around the house, almost totally without food. She was very lethargic, depressed and wouldn't touch food (totally unlike her). So I got worried and asked my nurse neighbor with 4 cats to look at her. She had a very rapid heartrate (like 250), so I called a 24 hour vet.

They saw her immediately. Her temp was 104.5, so he gave her fluids and then a shot of antibiotics, which he also gave me some to take home. When he felt around her abdomen before sending us home he got very concerned with what he felt. He said something didn't feel right, so he took X-rays and found her liver was 3 times it's normal size and pushing up against her stomach. He immediately said he thought it was cancer and that the diagnosis wasn't good. Basically telling me she wouldn't last more than a couple of weeks like this. I was devasted. He gave me the option of having ultrasound, but said it wouldn't really change the outcome and there wasn't anything that could be done.

I took her the next day for a second opionion and all he did was feel around and say that any kind of mass like that in that area (which he felt) wasn't a good sign.

She ate a little that afternoon, just nibbles of soft treats and some tuna. So I decided to take her back to doc #1 to have him give her more fluids and see what they could do temporarily to make her comfortable. They gave her a B12 shot and cortizone to help with immflamation. That night I got some fancy feast moist food and she has since been gobbling it down. She is still rather lethargic, but getting back to being more her normal self and eating great. She isn't drinking at all, but I was told with the sub-fluids they gave her twice and wet food she wouldn't be thirsty. She hasn't has a bowel movement since this all started (night before last), but she has peed.

The odd thing is her lab results came back today and it's all completely within normal limits. Said her liver functions are a smidge high, but nothing to even think twice about. She is also not jaundice at all.

We are probably going to proceed with an ultrasound and needle biopsy in a few days.

I was hoping you might have some thoughts as to what this could be?
Also curious if it's at all possible that she was born with a larger liver and since she has never had an X-ray we just had no idea? It would explain a lot of her problems over the years? The docs don't believe it's fatty liver because of the shape of it, but he said it's possible...just not likely.

Two other symptoms that she has had is excessive thirst and drooling.

Is it a mystery, should I be hopeful?

I haven't noticed her drooling since this all came about, but I haven't really been lounging with her as much because she still wants to be left alone a little more than usual.

What would that tell?

Excessive thirst can be symptomatic if Diabetes. Drooling can indicate dental desease. The significant thing here is that bacteria from decaying teeth can enter your cats bloodstream and affect her organs. Her liver may have enlarged only temporarily.
We had a client recently who took her cat to an after hours ER vet. They diagnosed her cat with advanced liver faiure. It turns out the cat had pancreatitis and the enlarged liver was only temporary. In addition, that particular cat is almost 17 years old and she has fully recovered.The ER vet is there for emergencies. He does not know your cat (or her history) personally).
Have your cats teeth and gums checked and have her evaluated for diabetes, pancreatitis and thryroid dysfunction. It is possible the steroid shot reduced the size of her liver.
I would take all these simple tests first. Then, if necessary, proceed to ultra sound and biopsy.
I sincerly hope your cat will have something easily treatable.And will recover quickly.

Reply to Terri Riba's Post: Thanks for your response. She had a full blood test done that came back normal, with a very slight liver function increase. The vet we called was a vet she had seen previously (two or three times) for her ears.

Would more tests need to be done to check diabetes, pancreatitis and thyroid function? It's been 4 days and she is still not drinking, but she is peeing a lot, which I can only assume is from the sub-fluids and water from her foods.

She is very bloated, almost totally round and appears to be in discomfort in her belly area, mostly when she tries to jump. She has had a BM once in the last 4 days.

Also, what is involved with having her teeth checked?

You will definitely want to check for diabetes, pancreatitis (can cause bloat and discomfort in belly area) and thryoid function. Teeth and gums are examined optically to ascertain if there are any infected or abcessed teeth that can release bacteria into your cats bloodstream.
SubQ fluids would hydrate your cat and reduce her need to drink but the effects should not last more then 36 hours.
You really need to get to the bottom of what is bothering your cat.
Tomorrow is not to soon to get this testing done.

Thank you for your help.

If it's any of the things you suggested why would her liver be so enlarged to the point she couldn't eat (and now can only eat very small meals frequently) The 3 vets that saw her didn't bring up any of these issues once they saw her liver?

Your cats liver could enlarge as a result of pancreatitis or diabetes. Also, not eating even for 24 hours can lead to enlargement and even liver failure. You will need to make sure your cat is not dealing with these issues (or hormonal Thyroid disfuntion) before you proceed to biopsy and needle aspiration. Hopefully, she can be cured with appropriate medication.
It is NOT symptomatic of cancer for the liver to enlarge and then return to almost normal size. So I am hoping your cat has an issue that is more easily resolved.

I think I am more confused than ever now. I am afraid of putting her through the biopsy (which the vet says is how we need to proceed) if it might do more harm than good??

If her liver was temporarily enlarged from not eating (or eating and vomitting) for a few days could it have gone down in a matter of 3 days? Should I re-Xray tomorrow when I take her for a clotting test before the biopsy (which will be 5 days since her first X-ray)? She has been eating really well for 3.5 days now.

She is acting almost like normal, except she is very uncomfortable looking....like from her swelling. She is sitting up in a position that takes any pressure off her tummy and she seems to have slightly labored breathing, especially after eating.
