Our 14 yr old male, yellow lab, has a cronic itching ...

Our 14 yr old male, yellow lab, has a cronic itching problem on the side of his face. He shakes his head so much he has developed a hematoma (sp?) twice, on seperate ears, that has required surgeries. We clean his ears, apply a topical spray (Gentocin), it gets better and then returns. A month ago he developed an itchy eye irritation. Now one of his ears is swollen again and our vet doesn't seem to have any answers for us. Can you give me some advice?
