my month old pound chihuahua has an elevated white blood

My 11 month old 6 pound chihuahua has an elevated white blood cell count (indicating infection) with normal bloodwork results. He is not eating or drinking and has a painful abdomen. He is lethargic. What could be wrong with him? What other tests should I have my Vet do?
