Horse mastitis

My horse has been weaned of her foal since the beginning of July. She has been getting mastitis and I treated her with Today seemed to go away for a period of time then it came back. I then treated her with SMZ for 10 days it went away and now it came back again. What do I need to do to get rid of it for good?
